Conditions of Entry

CT Heart Clinic Pty Ltd (ACN 635 893 113) (we, us or ours) is committed to protecting the health, safety and wellbeing of all our staff and patients (you). With a significant number of COVID cases still in NSW each day, we are maintaining the following Conditions of Entry into our premises. These Conditions of Entry apply in addition to NSW government requirements as applicable from time to time.

 

We kindly ask you not to enter our premises: 

  • unless you have an appointment with us (or accompanying someone with an appointment); 

  • if you refuse to wear a mask over your nose and mouth whilst inside our premises;

  • if you are awaiting for Covid-19 PCR results;

  • if you have fever, sore throat, shortness of breath, cough or loss of smell and taste or other Covid-19 symptoms;

  • if you have been diagnosed with Covid-19, unless at least 7 days have passed from your diagnosis, and you present to us evidence of your negative Rapid Antigen Test performed on your day of entry;

  • if you have been in close contact with a confirmed case of Covid-19 in the last 7 days, unless you present to us evidence of your negative Rapid Antigen Test performed on your day of entry; or

  • if you have been overseas in the last 7 days, unless you present to us evidence of your negative Rapid Antigen Test performed on your day of entry.

Upon entry to our premises, you agree to:

  • have your temperature checked by us;

  • present us with evidence of your negative Rapid Antigen Test if required by any of our conditions above;

  • if requested by us, perform a Rapid Antigen Test to confirm your negative result;

  • practice social distancing where possible; and

  • wash your hands or use hand sanitiser as directed by our staff.
