Conditions of Entry

CT Heart Clinic Pty Ltd (ACN 635 893 113) (we, us or ours) is committed to protecting the health, safety and wellbeing of all our staff and patients (you). These Conditions of Entry apply when you visit our premises.

 

We kindly ask you not to enter our premises: 

• unless you have an appointment with us (or accompanying someone with an appointment); 

• if you experience any signs or symptoms of a respiratory infectious illness (such as fever, sore throat, shortness of breath, cough or loss of smell and taste or other Covid-19 symptoms;

• if you are currently under isolation or restriction as directed by a health authority; 

• if you have been identified as a close contact of a confirmed case of a communicable respiratory disease and has not met the criteria for attendance as per current public health guidance; or

• if you decline to follow our respiratory infection control measures in place at the time (e.g., mask use, hand hygiene).

Upon entry to our premises, you agree to, if requested by us:

• have your temperature checked by us;

• wear a face mask covering your nose and mouth;

• present us with evidence of your negative Rapid Antigen Test;

• perform a Rapid Antigen Test to confirm your negative result;

• practice social distancing where possible; and

• wash your hands or use hand sanitiser as directed by our staff.
